Responsibilities

The On-call Registered Nurse is responsible for providing end-of-life care and hospice services to patients in their home or community setting. This includes intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in the provision of personal patient care. As an On-call Registered Nurse you will:

  • As scheduled, work in the office assisting with medical records and order processing along with other duties as needed
  • Assume primary responsibility for the patient/family caseload that includes assessing, planning, implementing, and evaluating care needs for patient care plan during on-call hours
  • Report On-Call activities to the RN Manager and RN Case Managers at the end of the call shift
  • Maintain and completes appropriate On-Call Log documentation as well as care coordination notes for phone calls and communication, visit notes, and other pertinent clinical information
  • As scheduled, attend interdisciplinary team meetings during on-call period for full team discussion, consultation and evaluation of patient/family needs

Qualifications

  • Currently licensed in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
  • Current driver’s license, proof of auto insurance, and reliable vehicle.
  • Two years recent experience as a professional nurse
  • Ability to work in a field setting and exhibited ability to make sound nursing judgments; must be able to work independently
  • Ability to assess patient needs and formulate individualized patient care plans to meet those needs
  • Knowledge of physical, psychosocial, and spiritual needs of terminally ill patients and their caregivers
