Responsibilities

The Patient Care Coordinator (PCC) focuses entirely on aiding the Provider in taking care of patients. As a Patient Care Coordinator you will:

  • Serve as the point person for the provider’s patients, facilities, and other healthcare professionals
  • Manage incoming calls and collect all pertinent medical information
  • Responsible for processing provider orders
  • Schedule consult and specialist appointments
  • Request medical records; review and assign faxes and attachments
  • Attend regular meetings and in-services, participates in committee meetings and professional development activities

Qualifications

  • Must be a Licensed Practical Nurse/Licensed Vocational Nurse
  • Must maintain a valid driver’s license and good driving record
  • Active CPR Certificate
  • Minimum 2 years of office experience in health care coordination
  • Basic working knowledge of medical terminology
  • Microsoft Office skills
  • Strong customer service, organizational, and interpersonal skills, with excellent multi-tasking and prioritization
  • Ability to communicate effectively with all personnel levels within the organization

Pay Range

USD $27.00 - USD $32.21 /Hr.
