Responsibilities

The Nurse Practitioner In-Home Assessment are responsible for performing Health Risk Assessments and occasional transitional care for patients discharged from hospitals to prevent re-admissions. As a Nurse Practitioner In-Home Assessment you will:

  • Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit
  • Educate and counsel patient and family based on assessment results, including recommendations
  • Performs follow up visits in transitional care

Qualifications

  • Current RN and NP licensure in the state of practice
  • Current CPR certification
  • Must be enrolled in both Medicare & Medicaid
